PCN Activity

By 2021 Primary Care Networks will be expected to deliver "locally agreed action to tackle inequalities," - one of seven specifications in the national service specifications. This will be achieved by a more concerted and systematic approach involving our practices the communities we serve and other partners. This approach is central to everything that is done by the NHS.
